- Submit a completed application through PharmCAS.
- Submit a SJC Supplemental Application and fee of $125 (non-refundable)
- Official PCAT exam scores
- Official transcripts from institution(s) where your degree(s) were conferred and where prerequisites were taken.
- 2 Professional letters of recommendation
